Code Instructions


How to respond:

  • ABCs: Always start with your primary survey (Airway à Breathing à Circulation)

  • If the patient is sitting up talking, it is automatically different from a stat response where the patient is altered with respiratory depression

  • Why was the stat response called?

  • What are the patient’s vitals?

  • What was the patient admitted for?

  • What has occurred during hospital stay?

  • Does this patient require immediate upgrade to ICU?

  • Do any labs need to be drawn? Does the patient require stat CXR or other imaging?

  • Discuss plan with staff. Determine who will follow-up on labs ordered and overall patient condition.

  • Stat response note:

  • Time of stat response called

  • Team members present at response

  • Reason for stat response

  • Patient’s vitals

  • Severity of patient condition

  • Interventions performed, labs/radiology/EKG/ABG ordered

  • Plan for following-up labs

  • Plan for escalation in level of care or monitoring on floor
